Back out the handle screw and look at the end of the stem. If the broach is a D shape rather than a round splined post, this extender indexes onto it.
After new tile, backerboard or a tub surround builds the wall out, the handle sits deep in the escutcheon, wobbles on the broach, will not seat flush, or cannot turn through its full range.
