Pull the handle and trim and look for a black plastic cap held to the valve body by four screws on a square flange, with a flat square-cut plastic stem the handle slides onto and two ports on the underside. A black cap with a plastic stem takes this kit. A brass stem means the brass-stem cap instead.
The tub spout or showerhead keeps dripping with the handle off, water weeps around the handle or trim plate, the handle has gone stiff or jumps between hot and cold, or the mix drifts during a shower. Hardened O-rings and a worn stem bore on the old cap cause it.
